- Personal statement: A personal statement (500 words or less) that may include educational and career aspirations, challenges you faced in achieving a goal, or any personal attributes you believe will contribute to your success in the program.

- Three NM Clinical Schools Letter of Recommendation Forms (attached): Please provide this form to three references (must be academically or professionally related). Letters of recommendation are to be sent directly from your reference to NM Clinical Schools Registrar: schools@nm.org.
- Official e-transcripts from all colleges and universities attended. Transcripts must be sent directly from the institution to NM Clinical Schools Registrar: schools@nm.org. Note: If e-transcript not available from institutions, please request official transcripts to be mailed to: NM Clinical Schools, 541 N. Fairbanks Court, Suite 950, Chicago, IL 60611.

- $25 application fee* (if paying in form of personal check, cashier's check or money order) payable to Northwestern Memorial Hospital. Only one application fee is required if applying to multiple programs.
- TOEFL iBT test results (if applicable). Please read the TOEFL Policy.
- Foreign transcript evaluation from Educational Credential Evaluators, Inc. (if applicable). Required for all courses taken outside the US. Degree and prerequisite course equivalencies must be documented. Evaluation must be sent directly from ECE to NM Clinical Schools Registrar.
